An abscess may form in virtually all kinds of tissues and if it develops within the eye cavity, it is referred to as orbital abscess (OA). Se hela listan på eyewiki.org Initially, IV antibiotics might be administered, but if no improvement appears within 48h, surgical drainage of the orbit and the affected sinuses must be performed. In medial or medial-inferior SPA a transnasal approach is used, but in superior orbital abscess an external incision is required. However, a medial subperiosteal orbital abscess is the most common serious complication to occur. Surgical intervention is mandatory whenever antibiotic treatment fails. Most authors prefer open surgical procedures such as external ethmoidectomy, while others recommend transnasal endoscopic drainage as the first attempt at sinus decompression. CT-scan found a 8 mm large subperiosteal orbital abscess with no further complications.

Orbital infections from sinusitis were classified by Chandler more than four decades ago, and this classification is used today to describe the progression in severity from preseptal cellulitis to orbital abscess to retro-orbital infection involving the cavernous sinus (Table 33.1) .

We present a case of a 50-year-old woman with acute dacryocystitis that was complicated by posterior rupture of the lacrimal sac causing an orbital cellulitis with subsequent visual acuity of no light perception. Upon presentation, she was immediately started on broad-spectrum antibiotics and underwent surgical incision and drainage of the lacrimal sac abscess but never regained vision. A review of literature is also reported focusing on aetiology and The exact incidence of orbital complications due to sinusitis in children is unknown. However, a medial subperiosteal orbital abscess is the most common serious complication to occur. Surgical intervention is mandatory whenever antibiotic treatment fails. Most authors prefer open surgical procedures such as external ethmoidectomy, while others recommend transnasal endoscopic drainage as the A contrast‐enhanced CT scan of the orbits showed a 1.7‐cm retro‐orbital abscess with no extension into the brain, confirming the diagnosis (Figure 1, C and D). After the diagnosis was made, the patient was given ampicillin/sulbactam and vancomycin intravenously, and the on‐call ophthalmologist was consulted. Also for the patients in whom a large orbital abscess causes discomfort, presence of superior or inferior orbital abscess, evidence of intracranial extension, involvement of frontal sinuses, and a known dental source of the infection in patients older than 9 years, urgent drainage usually within 24 hours has been recommended.

Conclusion: Orbital examination complicated by soft tissue oedemain children is not easy even If an orbital abscess requires drainage, an experienced orbital surgeon can generally evacuate the abscess under direct visualization at the time of sinus surgery. A transcaruncular conjunctival incision is the optimal approach to a subperiosteal abscess of the medial orbit.
